Claim Reporting Information

Please report all Workers’ Compensation claims within 72 hours (or 24 hours in the vent of a major injury or death) to avoid fines and penalties as determined by the State Department of Labor. Worker’s Compensation claims need to be reported on a WC-1 form. Please report all other claims including but limited to General Liability, Business Auto, and Property as soon as practical. Please make sure that your incident report includes at a minimum the following information; date of loss, location of loss, brief description of incident, claimant’s name & contact information, description of injuries (if any), description of damaged property, other insurance information, and policy report number (if any). We have also provided some generic incident reports for your use.

In the event of a lawsuit, prompt handling is critical due to the time constraints of the legal system for responding to complaints & summons. When a lawsuit is received, it should be promptly submitted to you Monarch Agent and/or Account Manager for further assistance for review and transmittal to the appropriate insurer for handling.

If a Monarch team member can not be reached and the incident requires immediate reporting, you can also use the direct reporting service offered by our carrier partners.
